To streamline your check-in process, please fill out your new patient forms ahead of time.

2: Bring Your Insurance Information
If you have dental insurance, please bring your card and any relevant information. As a courtesy, we will gladly submit your dental claims on your behalf.

3: Relax—You're in Great Hands
Our warm and welcoming team will greet you upon arrival. The doctor will then meet with you to review your radiographs and discuss any findings. If treatment is needed, we will provide a clear treatment plan along with an estimate.

Important Reminders

• Minors: Patients under 18 must be accompanied by a parent or guardian.
• Medical Conditions: Please inform us of any medical conditions that may affect your care (e.g., diabetes, high blood pressure, joint replacements, heart conditions, etc.).
